Patient retention: how to prevent treatment dropout and organize follow-up
Patient retention is lost between one session and the next: at the no-show that isn't rebooked, the package that runs out and the discharge with no follow-up. At a physical therapy teaching clinic, 59% of patients stopped before discharge. The answer is to give each of these moments a message, an owner and a deadline.
How many patients drop out of treatment?
There is no national number for private clinics. The published studies come from teaching clinics, and they show frequent dropout.
- Physical therapy. At a teaching clinic in Araçatuba (SP), 346 patients were followed from September 2013 to September 2016. At the end, 59.2% had dropped out of treatment and 31.8% were discharged, after an average of 14.6 sessions (UNIP, 2017).
- Psychotherapy. The article by Laskoski et al., in the journal Barbarói in 2020, reviews the literature. Early termination occurs in about one in five cases, and one review it cites ranges from 25% to 60%.
- The timing. In the same article, the risk of dropout is high in the first four sessions and falls sharply after the tenth.
The Araçatuba number comes from a single clinic, and it varies from place to place. The lesson works for any clinic: the start of treatment and the end of a package need attention.
At what moments do patients disappear?
Five moments deserve follow-up. Each has a signal in the schedule or in finance, and each calls for an action with an owner.
| Moment | Signal in the system | Action | Who | Deadline |
|---|---|---|---|---|
| First sessions | New patient with fewer than 4 sessions | Ask how the session went and confirm the next one | Provider | By the next day |
| No-show not rebooked | Status "No-show" with no new date | Call or message and rebook | Front desk | Same day |
| Package ending | 2 sessions left | Show the progress and propose the next step | Provider | Before the last session |
| Discharge | Treatment plan closed | Aftercare message with guidance and a way back | Front desk | 7 days after discharge |
| No return visit | 30 days with no session booked | Invitation for a reassessment | Front desk | In the week the deadline falls |
The deadlines in the table are an example. Adjust them to the type of treatment: a clinical Pilates plan has a different pace from weekly psychotherapy.
The point is to take these moments out of the front desk's memory. Each row becomes a list that someone opens every day.
How do you set up the follow-up routine?
Start with the two highest-volume moments, write the message for each and measure the response for four weeks. Then add the others.
- Choose the moments. A suggestion: no-show not rebooked and package ending, which the schedule and finance already show.
- Write the message with the provider. The aftercare message talks about the treatment, not a promotion. "Hi, Marina. It's been 30 days since your discharge. How is your knee? If you'd like a reassessment, reply here."
- Give it an owner and a deadline. Each contact has a person in charge and a date. Patients who haven't replied in two days go back on the front desk's list.
- Record the answer. "Rebooked", "not right now", "moved away". Without the reason, the clinic doesn't learn where it loses patients.
- Review every month. See how many contacts turned into sessions and change the message that doesn't work.
The channel needs the patient's opt-in. The WhatsApp Business policy, updated September 23, 2026, requires that the person gave their number and agreed to receive messages. It also recommends a separate opt-in for each type of message, for example session reminders and follow-up contact.
Ask for both opt-ins at registration, and record the date and the channel.
How do you keep patients engaged between sessions?
Give the patient something to do between sessions and the provider a way to see what they did. The gap stops being empty.
- Home exercises with video. The patient sees the right form and checks off what they did.
- Short questionnaire before the session. Pain scale from 0 to 10, sleep and difficulty with daily tasks.
- Review by the provider. Before the session, the provider opens the answers and adjusts the plan.
The clinical decision stays with the provider. The questionnaire organizes the information, and the person who treats the patient interprets the answers.
Digital contact with the care team is growing. According to Cetic.br's TIC Saúde 2025, online interaction with the team went from 16% of facilities in 2023 to 35% in 2025.
Which retention numbers should you track every month?
Five numbers are enough to see whether the routine works. Calculate each one on the right group of patients, not on the whole base.
| Metric | How to calculate | What it's for |
|---|---|---|
| Plan completion | Discharged patients ÷ patients who ended treatment in the month | Shows how many reach the end |
| Dropout in the first sessions | Patients who stopped before the 4th session ÷ new patients in the month | Points to a problem at intake |
| Rebooking after a no-show | No-shows rebooked within 7 days ÷ no-shows in the month | Measures the front desk routine |
| Package renewal | Packages renewed ÷ packages ended in the month | Links retention to revenue |
| Return after discharge | Discharged patients who came back within 90 days ÷ discharges in the period | Measures aftercare |
An example: the clinic had 50 no-shows in October and rebooked 30 within 7 days. Rebooking after a no-show was 60%. The other 20 become next month's list.
For no-shows, confirmation also counts. In the 2013 Cochrane review, attendance rose from 67.8% to 78.6% with a text reminder.
How CERNE solves it
CERNE connects the schedule, packages, records and the patient portal. Each moment in the table above becomes an alert or a task with an owner.
- The no-show becomes a task with an owner. A patient who misses goes into the "Rebook" column, with an owner and a deadline. The open slot goes to the waitlist, and CERNE follows up until the task closes.
- Packages with alerts. The system shows sessions used and remaining, and alerts before the package runs out.
- Portal between sessions. The patient sees the exercise videos, checks off what they did and answers the pain questionnaire. The provider sees the answers before the session.
- Aftercare. Patients discharged or with no visit in 30 days get the message on the routine the clinic approved. A reply becomes a booking, and no reply becomes a front desk task.
- AI assistant. Management asks "who has had no visit in 30 days?" and gets the list from operations data. The outreach only goes out with your approval.
CERNE's health package was born in the operation of Avittá Fisioterapia. In one year, the clinic gained more than 100 new clients, with good retention. See the workflows on the health and wellness page and the cards for each function.

Frequently asked questions
- How many patients drop out of physical therapy?
- There is no national number. At a physical therapy teaching clinic in Araçatuba (SP), 59.2% of 346 patients dropped out of treatment and 31.8% were discharged, between 2013 and 2016 (UNIP, 2017). It is data from a single clinic and does not work as an average.
- At what stage do psychotherapy patients drop out most?
- In the first sessions. An article in the journal Barbarói (2020) brings together studies that point to high risk in the first four sessions and a sharp drop after the tenth. Early termination occurs in about one in five cases.
- How do you do patient follow-up at a clinic?
- Define the contact points (no-show not rebooked, package ending, discharge, 30 days with no visit), write the message for each and give it an owner and a deadline. Patients who don't reply become a front desk task.
- Can I send follow-up messages to patients on WhatsApp?
- Yes, if the patient gave their number and agreed to receive messages. The WhatsApp Business policy, updated September 23, 2026, requires that opt-in and recommends a separate opt-in for each type of message.
- How do you measure the clinic's patient retention?
- Track every month the treatment plan completion rate, dropout by stage, package renewal and return within 90 days of discharge. Divide each number by the total patients in that group.
